The Consumer Coverage Disclosure Act requires all Illinois employers, including school districts, to provide the below disclosure/checklist identifying Essential Health Benefits (EHB) covered by the health plan(s). The document linked below applies to all Trust health plans currently being offered. Please note that coverage for the services listed remains subject to medical necessity and plan limitations may apply. Pre-authorization is required for certain services.
