Voluntary Dental Services, Optional by District

A dental program sponsored by the Egyptian Area Schools Employee Benefit Trust is available on a district-by-district basis. If you are enrolled in this program administered by Meritain Health you may visit any dentist at any time with no difference in benefits. However, it is to your advantage to select a dentist who is a dental preferred provider. If you select a dentist who is a dental preferred provider, the amount of out-of-pocket expense to you may be substantially lower. Life Insurance Program

A basic $10,000 life insurance policy is provided to members who participate in the Egyptian Trust health insurance program. Additional voluntary term life insurance coverage is available to district employees at very reasonable rates. Employees of participating districts may purchase up to $75,000 of additional term life insurance on a guaranteed issue basis (no medical underwriting) if purchased at the time the employee is first eligible to participate in the Egyptian Trust. Check with your local district representative to inquire about the voluntary life insurance program.

Voluntary Vision Plan

A vision plan sponsored by the Egyptian Area Schools Employee Benefit Trust is available on a district-by-district basis. This plan is offered through UniCare and is offered at very competitive rates. Information about the plan may be obtained at www.unicare.com

Flexible Benefits Plan Administration

The Egyptian Area Schools Employee Benefit Trust has endorsed American Fidelity Assurance Company to provide complete flexible benefit administration to member school districts at no cost to the employer or employee. The services provided by American Fidelity allow participating school districts to provide tax savings benefits to their employees on deductibles, copays, insurance premiums, and dependent care expense with absolutely no cost to the district or individual employee.
